Scan the GP e-Referral Form QR code.

Fill in your patient's details and the daignosis.

Submit the completed form

A copy of the e-Referral form will be emailed to your registered email.

Doctor may print a copy of the e-Referral form for your patient. Our Customer Service Team will be in touch with the patient to schedule an appointment.

The patient will receive a copy of the referral form in his / her email if they include their email address.
