Referring a patient

Refer a patient to Aurelius Hospital Pahang in easy steps

Download the referral form

Fill in your patient's details with diagnosis and submit the completed form (GP e-Referral).

A copy of the GP e-Referral form will be emailed to your registered email.

Doctor may print a copy of the e-Referral form for your patient. Our Customer Service Team will be in touch with the patient to schedule an appointment.

The patient will receive a copy of the referral form in his / her email if they include their email address.
